Referrals
Our communities offer a cost-effective alternative to hospital, and serve as the next step for people recently discharged. Our approach places relationships at the forefront of treatment. It empowers residents to actively participate in their recovery, reduces hospital admissions and helps them eventually move to lower support or independent living.
We accept referrals from the whole of the UK, from care coordinators, community mental health teams, social workers, hospitals or other professionals working directly with the person. Placements are typically funded through the resident's local authority or the NHS. If you are a family member or someone looking for support through our communities, please ask your clinical team to make a referral where possible.
To make a referral, please email our referrals team with supporting documents such as:
-
Recent risk assessment
-
Care Plan
-
Support Plan
-
Other information, including a psychiatric report, occupational therapist report or social circumstances report
FAQs
Who can be referred to CHT?
We accept referrals from the whole of the UK for adults aged 18 and above. All referrals are considered on an individual basis.
How long does the referral process take?
We aim to make the referral and assessment process as efficient as possible. Once we have received the referral and reviewed it clinically, we will arrange an assessment. The service manager or psychotherapist usually carries out the assessment; we will also invite you to visit the service separately. We then write an assessment report, which is shared with the care team and taken to a local authority or commissioner funding panel. Factors such as more complex referrals and delays from funding panel decisions might affect the length of the assessment process.
How are placements funded?
Placements are typically funded through the resident’s local authority or the NHS.
Can I refer to a specific community?
Yes, although we cannot always guarantee availability. If we cannot accommodate the referral at the chosen community, we will offer a space at an alternative community. Find out more about our communities.
How long is the average length of stay?
Everyone’s recovery journey is unique, so treatment timeframes vary. However, typically, placements last at least two years.
